During 2021, ARIKAYCE was approved in Japan and launched there; the company also continued commercial expansion in Europe. Its clinical-stage pipeline includes brensocatib and TPIP.

  • Cost of product revenue, excluding intangible amortization, was $44.2 million, or 23.4% of revenue, versus $39.9 million, or 24.3%, in 2020. On that basis, gross margin was approximately 76.6%, compared with 75.7%.
  • Research and development expense rose 50.6% to $272.7 million, mainly reflecting clinical trial spending. Selling, general and administrative expense rose 15.1% to $234.3 million.
  • At year-end, cash and cash equivalents were $716.8 million and marketable securities were $50.0 million. Working capital was $701.9 million. Management said available cash and securities were expected to fund operations for at least the next 12 months.
  • Outstanding convertible notes had $800 million face value: $225 million due 2025 and $575 million due 2028. Debt carrying value, net of discount and issuance costs, was $566.6 million. 2021 interest expense was $40.5 million.

Material changes versus the prior year

  • Revenue increased 14.6%, largely as Japan and additional European markets began contributing sales; U.S. revenue grew modestly.
  • Operating cash use increased by $144.0 million, principally due to greater research and development spending and working-capital movements.
  • R&D expense increased $91.6 million, including higher costs for the ARIKAYCE confirmatory program and brensocatib’s Phase 3 ASPEN trial. SG&A increased $30.7 million, including commercial expansion costs in Europe and Japan.
  • The August 2021 acquisitions of Motus and AlgaeneX added $29.6 million of in-process R&D, $136.1 million of goodwill, and $75.7 million of contingent consideration liabilities at year-end.

Outlook, commentary and risks

  • Management expected R&D expense to increase in 2022, primarily for ASPEN, ARIKAYCE’s ARISE and ENCORE trials, TPIP studies and other research. The company expected continued operating losses and stated that it may seek additional capital.
  • ARIKAYCE’s U.S. approval remains accelerated and depends on confirmatory evidence for full approval. Management anticipated ARISE enrollment completion in 2022 and top-line results in the first half of 2023; ENCORE enrollment was expected to finish by the end of 2023. The filing states confirmatory results are to be reported by 2024.
  • Brensocatib’s global Phase 3 ASPEN study was enrolling; enrollment completion was anticipated in early 2023. TPIP had three Phase 2 studies ongoing or planned, with preliminary data from a small number of PAH patients anticipated in 2022.
  • Key risks include ARIKAYCE market uptake, reimbursement and pricing; successful completion of confirmatory and pipeline trials; safety and regulatory outcomes; reliance on a small number of manufacturers and PARI as sole Lamira supplier; and access to capital. The three largest customers represented 75% of gross product revenue in 2021.
  • The company said COVID-19 did not materially affect 2021 financial results or operations, but future effects remained uncertain. Management did not expect ordinary-course legal proceedings to have a material adverse effect. No specific revenue, earnings or profitability guidance was provided.
