Product sales less reported cost of sales imply an approximate 93% product gross margin for the quarter; this is a simple calculation from reported figures, not a company-provided margin measure. Net loss increased year over year as commercial launch and corporate costs expanded. SG&A rose $36.5 million, primarily from commercial and market-research spending, additional personnel, and IT, legal and other corporate costs. R&D was broadly flat.

Material changes and operating developments

  • XDEMVY net sales were $24.7 million, up 89% from Q4 2023. The company reported approximately 26,000 bottles delivered to patients, up 65% sequentially, and approximately 8,000 eye care providers had initiated patients on treatment as of May 3, 2024.
  • Gross-to-net discounts were approximately 55%. Tarsus reported preferred-status contracts with two major commercial plans covering about 18 million lives, with expected benefits beginning in Q2 2024. It remained on track for broad commercial coverage by year-end 2024 and broad Medicare coverage beginning in 2025.
  • Q1 license and collaboration revenue included $2.5 million from a China-license termination payment and $0.4 million for cancellation of an unvested LianBio warrant. The prior-year quarter included a $2.5 million contractual milestone. The $2.9 million was therefore not solely recurring product revenue.
  • Operating cash use increased by $15.8 million year over year. Investing cash outflow was $102.2 million, principally reflecting purchases of marketable securities; financing provided $108.8 million, mainly from the March public offering.

Outlook, risks and unusual items

  • Management said the March 31 cash and investment balance was expected to fund current and planned operations for at least 12 months from the filing date. It expects continued operating losses and may need additional capital; runway depends on sales, expenses and debt availability.
  • On April 19, 2024, after quarter-end, Tarsus drew a $75 million initial tranche under a Pharmakon facility. A portion repaid the prior credit facility, leaving $39.8 million in net proceeds. The facility matures in 2029, bears interest at SOFR plus 6.75% with a 3.75% SOFR floor, and provides potential additional tranches subject to conditions, including sales milestones for later tranches.
  • Clinical pipeline updates: the company reported positive topline results for TP-04 in rosacea and TP-05 in a Lyme-disease-prevention study, and previously reported positive Phase 2a results for TP-03 in meibomian gland disease. It planned to discuss potential regulatory paths for these programs with the FDA by year-end 2024; further development and approvals remain uncertain.
  • Key risks include reliance on XDEMVY commercialization and payer access; uncertain market size and adoption; continued losses and possible future financing needs; dependence on third-party manufacturing, including a single-source lotilaner API supplier; clinical and regulatory uncertainty; and reliance on licensed Elanco intellectual property.
  • LianBio announced it would wind down; Tarsus transferred China Demodex blepharitis rights to GrandPharma under a March novation. Future milestones and royalties depend on development, patent and sales outcomes. The company reported no material litigation proceedings and no material loss contingency identified for recognition.
  • At quarter-end, five customers represented approximately 100% of accounts receivable; individual customers contributed 11%–27% of Q1 product sales. This concentration creates collection and distribution exposure.

Important facts for investors to verify

  • Whether XDEMVY prescriptions, patient starts and net sales continue to grow, and whether reported launch indicators translate into sustained demand.
  • Actual payer coverage, timing and impact of the reported preferred-plan contracts, and gross-to-net discounts as access expands.
  • Recurring commercial revenue and product economics versus one-time license-related revenue; monitor royalties, rebates and other sales deductions.
  • Cash burn, operating-expense growth and the assumptions behind management’s at-least-12-month runway estimate.
  • Terms and availability of additional Pharmakon tranches, interest burden and any sales conditions; distinguish quarter-end debt from the post-quarter refinancing.
  • Progress and FDA feedback for TP-03, TP-04 and TP-05, and the status of GrandPharma’s China development and patent-dependent milestones.
