Revenue drivers. JAKAFI revenue rose 7.7% to $2.594 billion, reflecting volume and price increases; it remained the largest product and a significant revenue concentration. OPZELURA revenue increased to $337.9 million from $128.7 million, driven by patient demand, refills and broader payer coverage. ICLUSIG, PEMAZYRE and MINJUVI revenues were $111.6 million, $83.6 million and $37.1 million, respectively; ZYNYZ generated $1.3 million in its first year of sales.

Profitability and expenses. Operating margin was approximately 16.8% of revenue and net margin approximately 16.2%. Total costs and expenses rose to $3.075 billion from $2.815 billion. Research and development expense increased 2.6% to $1.628 billion, while selling, general and administrative expense rose 15.9% to $1.161 billion, in part reflecting expansion of dermatology commercialization and OPZELURA promotion. Higher interest income and a $43.9 million unrealized gain on long-term investments also contributed to the increase in net income. The effective tax rate was 28.4%, compared with 35.6% in 2022.

Cash, liquidity and debt. Cash, cash equivalents and marketable securities totaled $3.656 billion at year-end, versus $3.239 billion a year earlier; working capital was $3.405 billion. Operating cash flow declined substantially, primarily due to working-capital changes, including higher inventory and lower accounts payable. Capital expenditures were $32.5 million; operating cash flow less capital expenditures was approximately $464.0 million. The company had no borrowings under its $500 million revolving credit facility at year-end and reported compliance with its covenants. The facility matures in August 2024. Total liabilities were $1.592 billion; these include $212.0 million of acquisition-related contingent consideration and lease liabilities, rather than reported funded borrowings.

Material changes, outlook and significant matters

  • Product and royalty revenue growth was partly offset by milestone and contract revenue falling from $165.0 million to $7.0 million. The prior-year amount included $135.0 million of regulatory milestones and a $30.0 million upfront payment.
  • Management said available cash, operating cash flow and credit-facility availability should be adequate for capital needs for the foreseeable future. The filing provides no specific numerical revenue or earnings guidance.
  • In February 2024, after year-end, Incyte paid MorphoSys $25 million for exclusive global tafasitamab rights; the prior collaboration was terminated. Incyte expects to recognize all U.S. commercialization revenue and costs under the new arrangement. Xencor remains eligible for specified future milestones and royalties.
  • Axatilimab’s BLA for chronic GVHD was submitted to the FDA in December 2023. Ruxolitinib extended-release tablets received an FDA complete response letter in March 2023; the company reported agreeing with the FDA in December 2023 on requirements to address it.
  • Incyte is contesting CMS’s treatment of OPZELURA as a Medicaid “line extension” of JAKAFI. It accrued approximately $59.5 million for potentially incremental rebates; the estimated impact was about 6.5% of OPZELURA gross-to-net deductions for the fourth quarter. A favorable outcome could reverse the accrual.
  • A separate contractual dispute with Novartis affects U.S. JAKAFI royalties. At year-end, $375.6 million of accrued royalties was included in accrued and other current liabilities, with payment dependent on the dispute’s outcome.
  • Key risks include reliance on JAKAFI, pricing and reimbursement pressure, competition and generic challenges, clinical and regulatory uncertainty, and reliance on third-party manufacturers and specialty distribution. The filing identifies generic challenges to JAKAFI and OPZELURA patents. It also notes dependence on Chinese suppliers for certain ruxolitinib raw materials, while additional European suppliers are being qualified.
  • Incyte reported an unqualified audit opinion and effective internal control over financial reporting. The auditor identified Medicaid rebate allowances as a critical audit matter because of estimation uncertainty and complex calculations.

Important facts for investors to verify

  • Whether JAKAFI can sustain growth and how much future revenue depends on it relative to newer products, particularly OPZELURA.
  • The outcome and financial impact of the CMS OPZELURA rebate litigation and the Novartis royalty dispute, including the timing and settlement of the $375.6 million accrual.
  • Whether operating cash flow recovers after the working-capital-driven decline and whether inventory levels convert to sales as expected.
  • Progress and regulatory outcomes for axatilimab, ruxolitinib XR and other late-stage programs, including the costs and obligations under the tafasitamab and collaboration agreements.
  • Potential effects of patent challenges, reimbursement changes, safety warnings and manufacturing or supplier disruptions on product revenue and margins.
