Q3 revenue rose about 11.6% year over year, while net income rose about 51.9%. For the nine-month period, revenue increased about 8.7% and net income about 27.0%; operating income declined about 14.9%. The Q3 operating improvement included substantially higher interest income than the prior-year quarter, partly offset by a $26.7 million unrealized loss on long-term investments.
Q3 product revenue was $783.2 million, led by JAKAFI at $636.3 million and OPZELURA at $91.8 million. OPZELURA revenue grew from $38.1 million in Q3 2022; management attributed growth to patient demand and expanded coverage. Product royalties were $130.8 million. Nine-month milestone and contract revenue was $5.0 million, compared with $135.0 million in 2022, which included significant Novartis and Lilly milestones.
Research and development expense was $375.7 million in Q3 and $1,183.1 million year to date; nine-month expense increased 9.1% year over year, reflecting continued late-stage development investment and increased development headcount. Nine-month selling, general and administrative expense increased 18.9%, primarily due to increased headcount and OPZELURA vitiligo launch promotion. The filing does not present a single consolidated gross-margin figure.
Cash, liquidity and debt
At September 30, 2023, cash and cash equivalents were $3.227 billion and marketable securities were $289.2 million, for combined cash and marketable securities of approximately $3.516 billion. Current assets were $4.403 billion and current liabilities were $1.125 billion. Total liabilities were $1.457 billion; the balance sheet reports finance lease liabilities of $31.9 million and acquisition-related contingent consideration of $207.0 million. Incyte had no borrowings under its $500 million revolving credit facility and reported compliance with its covenants.
Operating cash flow for the first nine months was $348.8 million, down from $686.3 million in 2022, primarily due to working-capital changes. Cash and cash equivalents increased from $2.951 billion at year-end 2022 to $3.227 billion. Cash, cash equivalents and restricted cash plus investments totaled $3.229 billion at period end, as presented in the cash flow statement. Management stated that operating cash flow, available cash and securities, and the credit facility should meet capital needs for the foreseeable future.
Material changes, outlook and risks
- Product revenue growth was driven in part by JAKAFI, whose nine-month sales increased to $1.899 billion from $1.762 billion, and OPZELURA, whose nine-month sales rose to $228.6 million from $67.5 million. JAKAFI remains a significant revenue concentration and a central business risk.
- Incyte accrued $341.5 million in royalties dependent on the outcome of a contractual dispute with Novartis over U.S. JAKAFI royalties. The filing reports $88.0 million of such royalties for the first nine months of 2023.
- Incyte is challenging a CMS interpretation that could treat OPZELURA as a JAKAFI “line extension” for Medicaid rebate purposes. It accrued approximately $42.9 million; the estimated effect on Q3 OPZELURA gross-to-net deductions was 6.6%. A favorable outcome could reverse the accrual and reduce future deductions.
- In September, Incyte received notice of an ANDA seeking approval for generic OPZELURA and challenging certain cream patents. The company also disclosed an earlier generic JAKAFI challenge; it reported no FDA action on either application to its knowledge.
- The FDA issued a complete response letter for once-daily ruxolitinib extended-release tablets in March 2023, identifying additional approval requirements. Incyte said it would work with the FDA on next steps. Two Phase III LIMBER studies of ruxolitinib plus parsaclisib were discontinued after interim analyses indicated they were unlikely to meet their primary endpoint; the decision was not attributed to safety.
- Positive Phase III axatilimab AGAVE-201 results were reported in July 2023, including a 74% overall response rate at the 0.3 mg/kg dose every two weeks. Incyte also initiated a Phase I study of INCA033989.
- Incyte reported that the expected $50 million purchase of property from Wilmington Friends School would not proceed and wrote off approximately $5.6 million in SG&A expense. The nine-month results also include volatile investment fair-value changes and a $14.1 million loss from remeasurement of acquisition-related contingent consideration.
- Key stated risks include reimbursement and pricing pressure, dependence on JAKAFI and a limited number of specialty distributors, clinical and regulatory setbacks, competition and generic entry, reliance on collaborators and third-party manufacturers, and investment-value fluctuations. The filing provides no numerical revenue or earnings guidance.
