Operating cash flow less capital expenditures was approximately negative $0.1 million in Q1 2019, compared with negative $2.8 million in Q1 2018. Management attributed the 2019 operating cash generation primarily to earnings and noncash expenses, partly offset by working-capital outflows, including payment of prior-year bonuses.

At March 31, 2019, total current assets were $195.5 million and current liabilities $42.8 million, implying working capital of $152.7 million and a current ratio of about 4.6. Debt principal outstanding was $269.5 million; the balance sheet reported $266.1 million of debt and other borrowings, net of discounts and issuance costs and including finance leases. The term debt carried a 6.2% rate, with $261.9 million of principal scheduled for 2022. The $75 million revolving facility had no balance outstanding; availability may be constrained by credit-agreement covenants.

Changes versus prior comparable period

  • DEFINITY revenue rose 14.5% to $51.1 million, principally on higher U.S. volume. Management described it as the company’s fastest-growing, highest-margin commercial product.
  • TechneLite revenue increased 12.9% to $24.1 million, reflecting annual pricing adjustments and volume; the prior-year comparison was affected by a temporary supplier disruption.
  • Other nuclear product revenue fell 22.4% to $15.1 million, including lower Xenon volume. Rebates and allowances also increased.
  • U.S. segment revenue grew 5.5% to $75.4 million; International revenue was nearly flat at $11.1 million. Operating income increased in both segments.
  • Research and development expense increased 23.6% to $4.9 million, primarily from DEFINITY clinical studies and higher employee costs. Interest expense rose approximately $0.5 million, attributed to higher debt interest rates.
  • The effective tax rate declined to 22.1% from 31.6%, primarily due to increased tax benefits from stock-compensation deductions. Other income included $0.8 million of tax indemnification income.

Outlook, risks and contingencies

  • No specific consolidated revenue or earnings guidance was provided. Management expects DEFINITY to continue growing and represent a larger share of product mix in 2019, and stated that available cash, operating results and revolver availability should fund requirements for the foreseeable future.
  • Supply of molybdenum-99 (Moly), used in TechneLite, remains a material risk. NTP’s latest processing outage began in April 2019, after quarter-end; the company reported that it could not meet all TechneLite demand on certain manufacturing days during outages. New ANSTO capacity was approved for the company’s use but was still ramping; SHINE supply was estimated to begin in 2021, subject to operational and regulatory approvals.
  • DEFINITY’s composition-of-matter patent expires in June 2019, while a method-of-use patent runs to March 2037. The company reported no generic applicant notice as of the filing date but warned of potential generic competition. A modified formulation could be available in 2020 if approved; in-house manufacturing capability was expected by early 2021, neither timing assured.
  • JHS is the sole-source manufacturer for DEFINITY and several other products. The company is pursuing supply-chain diversification and facility investment, but those efforts may not succeed on schedule. China DEFINITY approval was delayed by packaging-component and clinical-data issues.
  • Clinical development and regulatory approval for DEFINITY’s LVEF indication, flurpiridaz F 18 and LMI 1195 involve substantial cost and uncertainty. Management noted ongoing Phase 3 work and regulatory dependencies; no approval or commercial outcome is assured.
  • The company is pursuing monetary damages in arbitration with Pharmalucence over an unsuccessful DEFINITY manufacturing and supply arrangement; outcome and any recovery are uncertain. A $28.2 million surety bond provides required financial assurance for facility decommissioning; the recorded asset-retirement obligation was $11.9 million.
  • Management identified competition, reimbursement, third-party manufacturing and customer dependence, regulatory compliance, indebtedness and product-development execution among the principal risks. It reported effective disclosure controls and no material changes in internal control during the quarter.

Investor verification points

  • Track DEFINITY volume, pricing, rebate levels and the timing or impact of potential generic competition following the June 2019 patent expiration.
  • Verify Moly supply continuity, the operational ramp of ANSTO’s new facility, and any resulting TechneLite sales or margin effects.
  • Assess whether capital spending supports the planned North Billerica manufacturing capability and whether the project meets its stated timing.
  • Monitor debt repayment and covenant headroom, especially given the large 2022 principal maturity and constrained revolver availability.
  • Follow clinical-trial, regulatory and China-approval milestones, as well as the Pharmalucence arbitration; outcomes and financial effects remain uncertain.
