Product revenue included $133.9 million from ELEVIDYS, launched in June 2023. PMO product revenue was $225.5 million, compared with $231.5 million in the prior-year quarter. EXONDYS 51 revenue declined 9% to $120.2 million; AMONDYS 45 increased 9% to $71.9 million; and VYONDYS 53 increased 1% to $33.5 million.

Reported cost of sales was $50.6 million, producing an approximate reported gross margin of 87.8%. The approximate operating margin was 8.4% and net margin was 8.7%. These margins benefited from previously expensed manufacturing costs for much of the ELEVIDYS inventory sold. Management stated that incremental ELEVIDYS inventory costs would have been approximately $23.8 million higher if those costs had not previously been expensed in research and development.

Research and development expense was $200.4 million, down 18%, primarily because commercial ELEVIDYS manufacturing costs were capitalized after approval. Selling, general and administrative expense increased 15% to $127.0 million. Stock-based compensation expense was $40.7 million.

Liquidity and capital structureMarch 31, 2024
Cash and cash equivalents$427.3 million
Short-term investments$963.5 million
Non-current investments$33.9 million
Restricted cash$15.6 million
Total cash, cash equivalents and investments$1.440 billion
Convertible debt carrying value$1.239 billion
Working capital$1.856 billion

Total cash, cash equivalents and investments declined from $1.692 billion at December 31, 2023, primarily reflecting operating cash use and investment activity. Management believes available liquidity is sufficient to fund the current operating plan for at least the next twelve months. The remaining convertible notes include approximately $105.8 million of 2024 Notes and $1.150 billion principal amount of 2027 Notes.

Material changes versus the prior comparable period

  • Revenue increased $160.0 million, driven principally by the first full quarter of ELEVIDYS sales.
  • Roche collaboration revenue increased by $26.0 million. Sarepta recognized $48.0 million after Roche declined an option relating to an early-stage Duchenne program; $5.8 million of ELEVIDYS contract manufacturing revenue was also recognized.
  • Operating results improved substantially because Q1 2023 included a $387.3 million loss on debt extinguishment related to the exchange of 2024 Notes.
  • Operating cash use increased despite reported profitability, largely due to a $145.0 million increase in manufacturing-related deposits and prepaids, a $60.5 million inventory increase and a $50.4 million reduction in deferred revenue.
  • Total inventory increased to $581.1 million from $514.2 million at year-end 2023, including $207.5 million classified as non-current.
  • Manufacturing-related contractual commitments totaled $1.266 billion, including $836.0 million due from April through December 2024.

Guidance, outlook, commentary and risks

The filing does not provide a specific full-year revenue or earnings guidance range. Management expects to meet with the FDA regarding SRP-5051 in the second half of 2024. The FDA priority-review goal date for the ELEVIDYS efficacy supplement, which seeks to remove age and ambulatory restrictions, was June 21, 2024. Sarepta also began screening a Phase 3 SRP-9003 study for limb-girdle muscular dystrophy in January 2024.

All four commercial products received accelerated approval and remain subject to post-marketing requirements and confirmatory studies. Failure to verify clinical benefit, adverse safety findings, manufacturing deficiencies or unfavorable regulatory decisions could limit, alter or withdraw approvals. ELEVIDYS also faces substantial manufacturing, supply, reimbursement, patient-access and market-adoption risks.

  • Revenue remains concentrated in U.S. product sales and a limited customer base; two customers represented 36% and 20% of Q1 product revenue.
  • Sarepta relies on third-party manufacturers, including Thermo Fisher, Catalent and Aldevron. One source currently supplies ELEVIDYS drug substance and drug product, with an additional source under qualification.
  • Future development, regulatory, commercial and royalty milestone obligations under license and collaboration agreements could total up to $3.2 billion, excluding certain unexercised options.
  • Pending intellectual-property litigation includes Regenxbio and University of Pennsylvania claims concerning ELEVIDYS manufacturing and Nippon Shinyaku litigation involving exon-53 technology and VYONDYS 53. The filing states that no material litigation loss was accrued and no material loss range was estimable as of March 31, 2024.
  • The company continues to report an accumulated deficit of $4.410 billion and negative operating cash flow, notwithstanding Q1 profitability.
  • Management disclosed that internal disclosure controls were effective as of March 31, 2024, with no material changes to internal control over financial reporting.
