GAAP results were adversely affected by a $1.8 billion impairment of the Company’s VillageMD equity investment, recorded in net realized investment losses. Net realized investment losses totaled $1.836 billion versus $56 million in the prior-year quarter. The effective tax rate increased to 368.4% from 18.4%, primarily because of a $427 million valuation allowance related largely to the equity-security impairment.

Adjusted income from operations benefited from higher earnings in both major operating segments. Evernorth’s pre-tax adjusted income increased 3% to $1.360 billion, while Cigna Healthcare’s increased 20% to $1.340 billion. Cigna Healthcare’s adjusted margin improved to 10.1% from 8.8%, and its medical care ratio improved to 79.9% from 81.3%. Evernorth’s adjusted margin declined to 2.9% from 3.6% because of client onboarding investments, including Centene, and investments in digital and care solutions.

  • Total debt was approximately $32.8 billion, consisting of $1.715 billion of short-term debt and $31.053 billion of long-term debt, versus approximately $30.9 billion at year-end 2023.
  • The debt-to-capitalization ratio increased to 44.3% from 40.1%, primarily reflecting debt issuance and share repurchases.
  • The Company issued $4.5 billion of senior notes in February 2024 and repurchased $1.8 billion of existing senior notes through tender offers.
  • Operating cash flow was $4.840 billion; investing activities used $495 million and financing activities used $2.529 billion.

  • In April 2024, revolving credit commitments increased from $5.0 billion to $6.5 billion through a $5.0 billion five-year facility and a $1.5 billion 364-day facility. The Company had no borrowings under its revolving facilities at March 31.

Material changes versus the prior comparable period

  • Revenue growth was driven primarily by higher Evernorth pharmacy volumes, including the Centene collaboration beginning January 1, 2024, branded-drug inflation and Specialty and Care Services growth.
  • Pharmacy and other service costs rose 32%, broadly reflecting the increased pharmacy revenue base and Centene onboarding.
  • Cigna Healthcare profitability improved due to pricing execution, affordability initiatives, business mix and expense discipline, despite a 1% decline in medical customers to 19.2 million.
  • Unpaid claims and claim expenses increased 15% from December 31, 2023 to $5.863 billion. The increase reflected claim submission and payment disruptions related to a third-party cyber incident and normal stop-loss seasonality.
  • The Company’s equity securities declined to $1.571 billion from $3.362 billion at year-end, primarily because of the VillageMD impairment.
  • Other comprehensive loss increased to $460 million from $111 million, mainly due to changes in discount rates for long-duration insurance liabilities.
