Pharmacy revenues increased 7% to $137.2 billion, premiums increased 11% to $44.2 billion, and fees and other revenues increased 16% to $12.6 billion.
GAAP earnings were reduced by a $1.5 billion pre-tax estimated loss associated with the planned sale of Medicare-related businesses, compared with a $1.7 billion pre-tax gain on the 2022 Chubb transaction. The effective tax rate fell to 2.6% from 19.2%, primarily due to foreign deferred tax benefits and favorable tax developments.
Segment results and margins
- Evernorth Health Services: Adjusted revenue was $153.5 billion, up 9%; pre-tax adjusted income was $6.4 billion, up 5%. The pre-tax adjusted margin declined to 4.2% from 4.4%. - Other Operations: Adjusted revenue fell 74% to $596 million and pre-tax adjusted income fell 81% to $96 million, primarily reflecting the absence of businesses sold in 2022.

Principal risks
- Medical and pharmacy cost inflation, utilization changes, inaccurate claims reserves, pricing pressure and unfavorable medical cost trends could compress margins.
- Government regulation and reimbursement changes affecting Medicare, Medicaid, ACA products, pharmacy benefit managers, drug pricing, rebates, utilization management and minimum medical loss ratios could materially affect results.
- CMS Star Ratings, risk-adjustment audits and RADV proceedings may affect Medicare revenue, bonus payments, enrollment and potential repayments.
- Cyberattacks, privacy breaches, technology failures, artificial intelligence errors and third-party vendor disruptions could cause operational losses, regulatory penalties and reputational damage. Management states that no cybersecurity threat has materially affected or is reasonably likely to materially affect the company as of the filing date.
- The company has substantial indebtedness, including approximately $30.9 billion of total debt, and faces interest-rate, refinancing and credit-rating risks.
- Goodwill and other intangible assets totaled approximately $75 billion, or 49% of consolidated assets, creating impairment risk if projected cash flows, growth or valuation assumptions deteriorate.
- Investment risks include interest-rate volatility, credit deterioration, private-market valuation uncertainty, commercial real estate exposure and the concentration of strategic investments in health care.
