FDA Expands Eli Lilly's Blood Cancer Drug Label in First-Line Setting

Eli Lilly LLY announced that the FDA has expanded the label of its oral cancer drug Jaypirca (pirtobrutinib) to include a new indication in chronic lymphocytic leukemia or small lymphocytic lymphoma (CLL/SLL).

Jaypirca has been approved to treat adults with previously untreated CLL/SLL who do not have a known 17p deletion. With this green light, the drug can now be used as a first-line treatment for eligible patients.

The approval is supported by data from the phase III BRUIN CLL-313 study, which showed that Jaypirca reduced the risk of disease progression or death by 80% compared to chemoimmunotherapy.

Before the FDA’s latest approval, Jaypirca was already approved to treat adults with relapsed or refractory (R/R) CLL/SLL who have been previously treated with a covalent BTK inhibitor. The drug is also approved to treat adults with R/R mantle cell lymphoma (MCL) following two lines of systemic therapy, including a BTK inhibitor.

Jaypirca is already generating meaningful sales for Lilly, with revenues rising 66% year over year to $357 million in the first half of 2026. While the strong growth underscores the drug’s potential as an expanding oncology asset, its sales remain relatively small compared with Lilly’s much larger obesity and diabetes portfolio.

What Separates Jaypirca From the Competition

In the CLL/SLL market, Jaypirca competes with other established BTK inhibitor therapies, including Brukinsa, Calquence and Imbruvica. While Brukinsa is marketed by BeOne Medicines (formerly BeiGene), Calquence is sold by AstraZeneca AZN. Imbruvica is marketed by Johnson & Johnson JNJ in partnership with AbbVie ABBV. Like Lilly’s drug, all these products are available as oral therapies.

However, Jaypirca stands out because of its differentiated mechanism of action. Per Lilly, it is the first and only FDA-approved non-covalent BTK inhibitor. In simple terms, covalent BTK inhibitors like Brukinsa, Calquence and Imbruvica bind irreversibly to a specific part of the BTK protein called C481, thereby blocking its activity. Jaypirca works differently by binding reversibly to BTK without depending on C481. As a result, it can continue to bind to BTK even when C481 mutations develop, potentially making covalent BTK inhibitors less effective. This makes Jaypirca particularly relevant for patients whose disease has progressed following treatment with a covalent BTK inhibitor.
