  • Product revenue increased about 6%; total revenue rose about 1%. DEXTENZA generated all product revenue. Product cost was $1.214 million, implying an approximate product gross margin of 91% before operating expenses.
  • Cash and cash equivalents were $79.026 million at quarter-end; cash, cash equivalents and restricted cash totaled $80.790 million. Cash declined $23.274 million during the quarter, including $3.379 million of capital expenditures.
  • Management estimates existing cash will fund planned operations, debt service and capital expenditures into mid-2024, excluding planned pivotal OTX-TKI trials, which it does not intend to start without additional funding. The estimate depends on assumptions about DEXTENZA sales, spending and development pace.
  • Debt included $25.0 million principal under the fully drawn credit facility and $37.5 million principal of 2026 convertible notes, plus $9.191 million accrued note interest. The credit facility rate was 11.8% at March 31; interest-only payments run through May 2024, with principal repayment scheduled in 2024–2025. The convertible notes carry a 6% stated rate and mature in 2026 unless earlier converted, repurchased or redeemed.
  • Current assets were $107.162 million and current liabilities were $29.715 million. Stockholders’ equity fell to $9.711 million from $35.379 million at year-end 2022.

Changes, outlook and notable items

  • Operating expenses rose $4.903 million year over year, led by higher R&D, selling and marketing, and G&A spending. Net loss widened substantially, in part because a $6.563 million non-cash derivative remeasurement loss replaced the prior-year $6.958 million gain. The derivative liability increased to $12.914 million; its fair-value movement does not affect expected cash outflows.
  • Management reported more than 34,000 DEXTENZA billable units sold into the market in Q1, up approximately 25% from Q1 2022 and 8% from Q4 2022. These distributor-to-provider unit figures can differ from recognized revenue because of distributor stocking. Gross-to-net provisions were 28.1% of gross product sales, versus 21.9% a year earlier.
  • Management expected OTX-TKI wet AMD pivotal trial initiation as early as Q3 2023, subject to financing; a DR Phase 1 readout as early as Q4 2023 and possible Phase 3 initiation as early as Q1 2024, subject to data and financing. OTX-TIC Phase 2 topline results were expected in Q4 2023. These are forward-looking expectations, not assured outcomes.
  • OTX-TIC enrollment in the 5 μg Phase 2 arm was stopped after IOP elevations in six subjects; enrollment continued in the 26 μg and DURYSTA arms. The 2021 OTX-CSI Phase 2 trial did not meet its primary endpoint of increased tear production versus vehicle; formulation work continues.
  • In the U.S. OTX-TKI wet AMD Phase 1 interim results, 73% of treated subjects remained rescue-free through Month 10 and treatment burden was reported 92% lower than subjects’ historical injection regimens. The filing describes these as interim, early-stage findings; one endophthalmitis serious adverse event followed a protocol-required aflibercept injection and was assessed as procedure-related.
  • The company drew and repaid a $2.0 million short-term protective advance in March following Silicon Valley Bank’s closure. It amended its credit agreement to use SOFR-based interest provisions and, in May 2023, obtained permission to hold up to 50% of cash and equivalents at institutions other than SVB.
  • Cash remained concentrated at two financial institutions, with balances above FDIC insurance limits. Three specialty distributors represented 90% of Q1 gross product revenue and 92% of quarter-end accounts receivable. The company also relies on a limited number of third-party manufacturers.
  • AffaMed’s China clinical-trial authorization for DEXTENZA, announced in April 2023, triggered a $1.0 million milestone invoiced after quarter-end. No material legal proceedings or subsequent events were reported; the filing notes ordinary-course indemnification obligations whose maximum potential payments are not determinable.

Important facts for investors to verify

  • Whether DEXTENZA sales growth, reimbursement access and gross-to-net deductions support management’s cash runway assumptions.
  • Timing, financing and clinical results for OTX-TKI pivotal studies, and the upcoming OTX-TIC Phase 2 readout.
  • Cash burn, planned capital expenditures and ability to raise capital before the projected mid-2024 runway endpoint.
  • Credit facility covenants, interest costs, repayment schedule and the convertible notes’ accrued interest, conversion terms and derivative valuation.
  • Exposure to distributor concentration, manufacturing interruptions and uninsured cash balances.
