Operating loss margins are calculated from reported operating loss and revenue; the filing does not present a gross margin measure. Research and development expense fell 40% to $116.1 million, primarily reflecting pipeline prioritization and reduced staffing. Selling, general and administrative expense declined 16% to $73.3 million, while acquired-intangible amortization rose 31% to $51.5 million. Net interest expense increased 49% to $40.8 million, largely due to interest on the royalty-sale liability.
Revenue mix and drivers: Translarna revenue was $103.6 million, down 10% year over year, which management attributed mainly to timing of bulk patient orders. Emflaza revenue was $57.5 million, up 5%, driven by new patient starts and high compliance. Evrysdi-related royalty revenue was $31.2 million, slightly higher year over year. Manufacturing revenue was $1.4 million, down from $2.0 million.
Cash, liquidity and obligations
- Cash and cash equivalents were $548.4 million and marketable securities were $336.5 million at March 31, 2024, totaling $884.8 million. Cash and equivalents alone declined from $594.0 million at year-end 2023.
- Operating cash flow was positive $70.8 million, versus cash used of $29.5 million in Q1 2023. Management said the 2024 result primarily included a $100 million cash receipt for an Evrysdi sales milestone; operating cash flow should therefore be viewed in that context.
- Investing activities used $114.9 million, including product-rights acquisitions, fixed-asset purchases and marketable-security purchases. Financing activities provided $0.5 million. Cash, cash equivalents and restricted cash ended the quarter at $564.6 million.
- Long-term debt had a carrying value of $284.5 million, representing $287.5 million principal of 1.50% convertible notes due September 2026. The Blackstone term loan was repaid and terminated in October 2023.
- The liability for sold future Evrysdi royalties totaled $1.839 billion, including $227.1 million current and $1.612 billion noncurrent; its effective interest rate was 10.8%. This royalty-sale obligation is a significant source of reported interest expense.
- Total liabilities were $2.684 billion and stockholders’ deficit was $893.9 million. Accumulated deficit was $3.375 billion. Current assets were $1.169 billion and current liabilities were $574.5 million.
Material changes, outlook and risks
- Translarna: The EMA’s CHMP issued a negative opinion on renewal of its EEA conditional marketing authorization after re-examination. The European Commission had not adopted the opinion as of the filing; if adopted, Translarna would lose EEA authorization. PTC is exploring alternative access mechanisms and planned a U.S. NDA resubmission based on Study 041 and registry data by mid-2024. FDA feedback had stated Study 041 alone did not provide substantial evidence of effectiveness to support resubmission.
- Emflaza: U.S. orphan-drug exclusivity for patients aged five and older expired in February 2024. Management expects this to have a significant negative effect on revenue; exclusivity for patients aged two to under five runs through June 2026.
- Pipeline milestones: PTC submitted a U.S. BLA for Upstaza and an EMA MAA for sepiapterin in March 2024. It expected to submit a U.S. sepiapterin NDA no later than Q3 2024, subject to completion of a 26-week mouse carcinogenicity study. PTC expected PTC518 12-month interim data and an FDA safety update in Q2 2024; the U.S. Phase 2 study remained partially on clinical hold. Utreloxastat Phase 2 topline results were expected in Q4 2024. Vatiquinone’s Phase 3 trial missed its primary endpoint; PTC reported FDA willingness to review a potential NDA using trial and open-label extension data, potentially in late 2024, while EMA advice indicated the data likely would not support conditional authorization.
- Funding outlook: Management expects cash flows from product sales and royalties, together with cash, cash equivalents and marketable securities, to fund operations for at least the next 12 months. PTC also expects continued significant expenses and operating losses and states it has never been profitable; it may need additional financing, which may not be available on acceptable terms.
- Potential payments: PTC anticipated up to $65 million in cash regulatory milestone payments to former Censa securityholders in 2024 for sepiapterin, subject to milestone achievement. It also disclosed Upstaza-related payments of $20 million upon FDA filing acceptance and $4.5 million upon BLA approval.
- Other reported items: Q1 2024 included $3.7 million of inventory write-downs, mainly for reserves and product nearing expiration. PTC reported no material legal proceedings and said disclosure controls were effective. A worldwide SAP ERP implementation occurred during the quarter; management said it did not materially affect internal control over financial reporting.

- Whether sepiapterin and Upstaza regulatory submissions progress on the stated timelines, including required studies, FDA review and related milestone payments.
- How much of operating cash flow reflects recurring operations versus the one-time $100 million Evrysdi milestone receipt, and the impact of PTC’s sale of future royalty rights on future cash flows and interest expense.
- Whether the $884.8 million of cash and marketable securities remains adequate against ongoing losses, the royalty-sale liability, development spending and potential milestone obligations.
- Clinical and regulatory outcomes for PTC518, utreloxastat and vatiquinone, including the partial U.S. clinical hold and vatiquinone’s missed primary endpoint.
