| Product revenue | $10.6 million | $12.8 million | Down 17% |
| Service and other revenue | $13.4 million | $7.8 million | Up 72% |

| Cash used in operating activities | $13.7 million | $16.2 million | Use decreased $2.4 million |
Biologics and drug-delivery revenue increased 49% to $13.6 million, principally from growth in services and license fees, partly offset by lower disposable-product sales. Functional neurosurgery navigation and therapy revenue decreased 7% to $8.5 million; capital equipment and software revenue decreased 21% to $1.8 million.
Gross margin declined mainly because preclinical services carried lower margins as the company expanded those offerings, and because of costs associated with its manufacturing-facility transition. Operating expenses rose: research and development to $11.7 million, sales and marketing to $12.6 million, and general and administrative to $11.8 million. Management cited staffing growth, investment in clinical and marketing teams, and a higher credit-loss allowance among the drivers.
Balance sheet, debt and liquidity
At December 31, 2023, cash and cash equivalents were $23.1 million, versus $27.6 million a year earlier; short-term investments were zero, versus $9.9 million. Total assets were $42.7 million, total liabilities $21.5 million, and stockholders’ equity $21.2 million. The company reported a cumulative deficit of $172.5 million.
A $10 million secured convertible note, carried at $9.9 million net of financing costs, is due in January 2025. It bears interest at the greater of three-month SOFR or 2%, plus 2%; management estimated approximately $0.8 million of interest expense over the next 12 months at then-current rates. The note is secured by company assets, and the noteholder is also a significant customer and related party.
Management stated that year-end cash was sufficient to support operations and obligations for at least 12 months based on its forecasts, while expecting continued operating cash consumption. A March 2024 public offering generated approximately $14.0 million in net proceeds. The auditor’s report includes an emphasis-of-matter paragraph concerning the company’s cumulative losses and going-concern assessment; the financial statements were prepared on a going-concern basis.
Material changes, outlook and risks
- Revenue growth was driven by biologics and drug-delivery services, while product revenue declined and losses widened. Operating cash use improved year over year.
- Management expects revenue growth from higher utilization at existing sites, new installations, pharmaceutical-partner sales and services, and strategic partnerships. It also expects higher operating expenses and continued operating cash consumption. The filing does not provide a specific numerical revenue or earnings forecast.
- The company completed its move from Irvine to its Carlsbad manufacturing facility in September 2023. Manufacturing is concentrated at a single site, and the company identifies single-source suppliers as a supply-continuity risk.
- Customer concentration is significant: one related-party pharmaceutical customer represented 12% of total revenue and 21% of biologics and drug-delivery revenue in 2023; the five largest hospital customers accounted for about 27% of functional-neurosurgery navigation disposable revenue.
- Four customers represented 28%, 26%, 16% and 10% of accounts receivable, respectively, at year-end. The allowance for credit losses increased to $1.4 million from $0.1 million.
- Growth in drug-delivery revenue depends partly on partners continuing research and trials and obtaining regulatory approvals. Product adoption also faces reimbursement, competition, regulatory-clearance and market-acceptance risks.
- Subsequent to year-end, the SmartFrame OR Stereotactic System received FDA clearance in January 2024 and entered limited market release in 2024. The March 2024 equity offering added liquidity but diluted existing shareholders.
- The company reported no material pending legal proceedings and stated that management considered disclosure controls and internal control over financial reporting effective as of year-end. The auditor did not provide an attestation opinion on internal control.
