Sales gross margin improved to 44.1% from 39.2%, primarily reflecting much lower semiconductor-chip premiums: less than $0.1 million in Q1 2024 versus $4.5 million a year earlier. Rental gross margin fell to 43.7% from 54.1%, reflecting lower revenue per patient, fewer patients billed as a share of those on service, a higher private-payor mix and rental revenue adjustments.
Cash and cash equivalents were $107.4 million and marketable securities were $12.4 million at March 31, 2024, totaling $119.8 million. Current assets were $199.2 million and current liabilities $73.9 million, giving working capital of $125.3 million. The balance sheet reports no borrowings; it does report operating lease liabilities of $22.1 million. Cash and cash equivalents declined $18.0 million during the quarter. Investing cash use was $13.4 million, including net purchases of marketable securities and spending on rental equipment and property.
Material changes and operating trends
- System sales increased approximately 26% to 33,900 units. Business-to-business sales rose in both domestic markets (31.3%) and international markets (37.2%); management attributed growth mainly to resellers, new customers and European partners.
- Domestic direct-to-consumer sales fell 15.6%, primarily due to lower sales-representative headcount. Rental revenue declined despite growth in patients on service, which reached approximately 51,800 versus 45,800 a year earlier.
- Total operating expenses declined to $50.6 million from $52.6 million. Lower sales and marketing and general and administrative expenses more than offset increased research and development spending.
- Net loss narrowed by $5.8 million, principally due to increased sales revenue, lower operating expenses and reduced material premiums.
Outlook, risks and contingencies
- No specific numerical revenue or earnings guidance is provided. Management says available cash, marketable securities and expected cash generation should cover projected operating and investing needs for at least the next 12 months, while cautioning that assumptions may prove incorrect.
- Management expects minimal semiconductor-chip cost premiums during 2024, but identifies inflation, wage and hiring pressures, and possible supply constraints as continuing risks. Growth also depends on sales productivity, product acceptance, reimbursement, and successful integration and U.S. regulatory clearance for Physio-Assist’s Simeox airway-clearance system.
- Rental revenue depends on reimbursement and billing factors, including the proportion of patients who are billable and the capped-rental period. Medicare represented 58.4% of rental revenue in Q1 2024, down from 73.8% a year earlier.
- The company disclosed that the Change Healthcare cyberattack in February 2024 caused connection issues with a third-party service provider and delayed rental collections. It also disclosed a prior data-security incident that resulted in remedial, legal and other costs; future incidents could disrupt operations or lead to claims, penalties and reputational harm.
- Physio-Assist acquisition consideration includes a potential cash earnout of up to $13.0 million, tied to U.S. regulatory milestones. The earnout liability was $10.6 million at March 31, 2024, after a $0.6 million fair-value increase during the quarter.
- Outstanding vendor purchase orders due within one year were approximately $75.5 million; $1.9 million was accrued for estimated losses on firm purchase commitments. Product warranty liabilities totaled $25.5 million.
- Inogen is party to ordinary-course legal proceedings and says it does not currently expect them to have a material adverse effect, while noting litigation can incur costs and divert management resources. The filing adds cybersecurity risk disclosures; otherwise, it reports no material changes to previously disclosed risk factors.
Important facts for investors to verify
- Whether business-to-business sales growth can offset continued weakness in direct-to-consumer sales, and whether sales-margin gains persist as product and channel mix changes.
- Whether rental revenue and rental margins stabilize as billing rates, payor mix, revenue adjustments and the capped-patient population evolve.
- Whether operating cash use improves and cash resources remain adequate while the company funds rental equipment, operations and purchase commitments.
- Progress toward U.S. regulatory clearance for Simeox and the resulting potential earnout payment.
- Further information on the cyber incidents, collection delays, remediation costs and protection of customer and health information.
- Concentration risks: three vendors accounted for 24.6%, 17.2% and 10.0% of Q1 raw-material purchases; one customer represented more than 10% of receivables at quarter-end.
