  • Q2 revenue increased 32.4% year over year; six-month revenue increased 26.9% (27.8% on a constant-currency basis, a non-GAAP measure). Omnipod revenue was $380.5 million for Q2 and $738.1 million for the first half. U.S. Omnipod grew 40.9% and 44.6%, respectively; international Omnipod grew 16.0% and 9.5% reported.
  • Drug Delivery revenue was $16.0 million in Q2, up from $13.6 million, but first-half revenue fell 58.2% to $16.5 million, primarily on lower partner demand.
  • Q2 gross margin improved 320 basis points, supported by pharmacy-channel mix, manufacturing efficiencies and lower warranty expense. First-half margin was broadly flat year over year.
  • Operating expenses rose: first-half R&D increased 22.8% to $105.2 million and SG&A increased 12.6% to $341.4 million. SG&A growth reflected staffing, customer support, software and commercial investments; prior-year legal costs partly offset the increase.
  • First-half operating cash flow was $44.5 million, versus $11.6 million. Investing cash use was $65.2 million and financing cash use was $8.9 million; cash, cash equivalents and restricted cash decreased $29.6 million to $660.1 million.
  • At June 30, cash and cash equivalents were $660.1 million; total debt, net, was $1,397.8 million, including $29.2 million current debt. The $300 million revolving facility had no borrowings outstanding. Debt-to-total-capital was 72%; net debt-to-total-capital was 38%.

Material changes and notable items

  • Q2 returned to profitability from a year-earlier loss; first-half net income was $51.1 million versus a $7.2 million loss. Lower net interest expense helped, primarily because higher interest income more than offset financing costs.
  • First-half operating cash flow benefited from earnings but was constrained by working-capital investment, including a $62.7 million inventory increase and higher receivables. Management attributed the inventory build to anticipated demand and launches.
  • Insulet recorded an $8.8 million first-half reduction in its estimated liability for 2022 voluntary medical device corrections (MDCs), benefiting cost of revenue. The remaining estimated MDC liability was $13.2 million at June 30, down from $54.6 million at year-end 2022.
  • First-half results also included $25.1 million paid for Bigfoot Biomedical patent assets. The company is building a Malaysia manufacturing facility, with production expected to begin in 2024.
  • Three distributors each represented at least 10% of revenue; together they accounted for 67% of Q2 revenue. Related-party revenue was $109.2 million in Q2 and $206.0 million for the first half.

Outlook, risks and management commentary

  • Management expects strong full-year 2023 U.S. Omnipod revenue growth, driven by Omnipod 5, pharmacy-channel growth and recurring Pod sales; it anticipates lower Omnipod 5 conversions in the second half compared with 2022.
  • Full-year international Omnipod revenue is expected to increase, with competition from automated insulin-delivery systems a partial offset. Drug Delivery revenue is expected to decline $26–$29 million for 2023.
  • Management forecasts full-year gross margin of 65%–66%, with manufacturing costs, Omnipod 5 production mix and inflation among the expected offsets to benefits from volume, sales mix and efficiencies. R&D and SG&A spending are expected to rise versus 2022.
  • Planned product and market developments include Omnipod 5 launches in Germany in 2023 and broader European rollout in 2024; a U.S. Omnipod GO pilot followed by a planned 2024 launch; and continued clinical and regulatory work, including an iPhone app submission. These plans are subject to execution, regulatory and reimbursement outcomes.
  • Management believes liquidity is sufficient for at least 12 months. Risks highlighted include reliance on Omnipod, reimbursement and competition, supply-chain and manufacturing scale-up, product quality and regulatory matters, international expansion, and the ability to service debt. Management reported no material changes to previously disclosed risk factors and said disclosure controls were effective.

Key facts for investors to verify

  • Whether Omnipod 5 adoption and pharmacy-channel growth sustain revenue momentum, particularly as conversions moderate and distributor inventory normalizes.
  • Whether projected 2023 gross margin and revenue outcomes are achieved, given inflation, manufacturing scale-up, product mix and the expected Drug Delivery decline.
  • Whether inventory growth converts to sales and operating cash flow, and how working-capital needs evolve.
  • How distributor and related-party concentration affect revenue, receivables and channel risk.
  • Progress and costs related to MDC replacements, Malaysia manufacturing, product launches, clinical trials, reimbursement and competition.
