Agios' Growth Story Hinges on Mitapivat's Commercial Expansion

Agios Pharmaceuticals AGIO is entering an important phase of its growth story as its lead therapy, mitapivat, expands across multiple blood disorders. The drug is marketed as Pyrukynd for pyruvate kinase (PK) deficiency and as Aqvesme for thalassemia in the United States. Outside the United States, mitapivat continues to be marketed as Pyrukynd for both indications. Agios is also seeking to expand mitapivat into sickle cell disease (SCD).

However, setbacks in its pipeline and growing competition in SCD have increased the company’s reliance on mitapivat’s commercial performance.

Mitapivat Products Drive Agios’ Top-line Growth

Agios’ commercial momentum accelerated in January 2026 following the U.S. launch of Aqvesme for adults with alpha- or beta-thalassemia. Pyrukynd and Aqvesme together generated $44.7 million in worldwide net revenues in the second quarter of 2026, up 259.3% year over year. The increase is particularly notable because Aqvesme was still in the early stages of its U.S. launch during the quarter.

Aqvesme showed early physician adoption, with 442 cumulative prescriptions from REMS-certified U.S. physicians as of June 30, 2026, compared with 242 at the end of the first quarter. Continued growth in prescriptions could support further patient starts and increase the drug’s contribution to Agios’ revenues.

The international opportunity is also expanding. In May 2026, the European Commission approved Pyrukynd for thalassemia, providing Agios with another avenue for increasing mitapivat sales outside the United States.

The next major opportunity for mitapivat is its potential entry into the SCD market. Agios submitted a supplemental new drug application for mitapivat in SCD under the accelerated approval pathway. The FDA has granted priority review to the filing, with a final decision expected by Nov. 1, 2026.

The discontinuation of tebapivat, Agios’ investigational next-generation PK activator after setbacks in lower-risk myelodysplastic syndromes (LR-MDS) and SCD in mid-2026, has removed a next-generation growth driver. As a result, AGIO is heavily dependent on mitapivat for its near- to medium-term commercial growth.

AGIO Faces Competition in Target Markets

Agios faces competition from established and emerging therapies across its target markets.

Novo Nordisk NVO is developing etavopivat, an investigational drug with a mechanism of action similar to that of Pyrukynd, which is being evaluated for SCD in late-stage development. Following encouraging late-stage data, Novo Nordisk plans to seek regulatory approval for etavopivat in the fourth quarter of 2026, potentially intensifying competitive pressure on Agios’ mitapivat program in SCD.

Agios also faces competition from therapies developed by major pharmaceutical companies, including Bristol Myers Squibb BMY, Merck, Pfizer, Novartis NVS and Vertex, as well as emerging treatments from smaller biotechnology companies. These companies are developing or marketing therapies for genetically defined and blood-related disorders that overlap with Agios’ target markets.

In thalassemia, one of the key competitors is Bristol Myers Squibb’s Reblozyl (luspatercept), an injectable erythroid maturation agent approved in the United States for anemia in adults with beta-thalassemia who require regular red-blood-cell transfusions, as well as certain adults with LR-MDS. Bristol Myers Squibb is expanding Reblozyl’s potential use. In February 2026, the company reported positive phase II results in alpha-thalassemia, including benefits in both non-transfusion-dependent and transfusion-dependent patients. If approved for SCD, Agios could face stiff competition from Novartis’ Adakveo, which is already approved for the said indication.
