Revenue and product mix: Product revenue was $1.536bn in Q3, up 62%, and $4.576bn for nine months, up 67%. TRIKAFTA/KAFTRIO contributed $960.3m in Q3 and $2.773bn year to date; its U.S. launch in late 2019 was the principal growth driver. Revenue from older CF products declined as some U.S. patients switched to TRIKAFTA, partly offset by ex-U.S. growth. Q3 ex-U.S. revenue was $313.7m, compared with $239.5m.
Margins and expenses: Q3 cost of sales was 12% of product revenue versus 14% a year earlier; reported operating margin was approximately 43.7% versus 10.5%. Q3 R&D and SG&A combined were $678.0m, down from $715.6m. R&D fell 11% in Q3, primarily due to lower collaborative payments, but increased 7% for the nine-month period as development spending grew. SG&A rose 16% in Q3 and 21% year to date, largely supporting launches and pipeline programs.
Profit quality: Q3 other income, net, was $84.4m, including $69.8m of unrealized gains on publicly traded equity investments. Nine-month other income, net, was $139.6m and included $102.3m of net unrealized gains on equity securities held at period-end, as well as realized gains. These investment gains and discrete tax benefits contributed to reported earnings and may fluctuate. Nine-month income tax provision was $120.7m, versus $124.4m; 2020 benefited from tax items including an intellectual-property transfer to the U.K.

Finance lease liabilities were $546.5m; a $268.5m operating lease liability was recorded for a new Boston facility.

Material changes, outlook and risks
- Q3 revenue rose sharply following the U.S. launch of TRIKAFTA and continued uptake in international markets. The European Commission approved KAFTRIO in August 2020; management anticipated a positive future revenue effect. No specific numerical revenue or earnings guidance is provided in the supplied filing text.
- Pipeline developments included positive Phase 3 data for the triple combination in children ages 6–11, with an FDA submission planned for Q4 2020; CTX001 trial updates were expected in Q4; and data for VX-864 and VX-147 were expected in 2021. These are expectations, not assured outcomes.
- In October 2020, after quarter-end, Vertex discontinued VX-814 development based on its Phase 2 safety and pharmacokinetic profile. In Q3 it entered a Moderna gene-editing collaboration, paid a $75m upfront amount, and expensed it as R&D.
- COVID-19 had not materially affected product demand or supply to date, according to management, but could disrupt trials, operations, reimbursement or pricing. Other key risks include clinical and regulatory uncertainty, reimbursement access, competition, and volatility in strategic equity investments. Management noted no material legal proceedings or material contingent liabilities accrued at quarter-end.
