Business context and reporting period
Perspective Therapeutics, Inc. (NYSE American: CATX) filed this Form 10-K for the year ended December 31, 2023. The company changed its fiscal year-end from June 30 to December 31; the December 2022 calendar-year comparative figures are unaudited. The company’s principal focus is developing lead-212 targeted alpha radiopharmaceuticals and companion imaging diagnostics. Its lead candidates, VMT-α-NET for SSTR2-positive tumors and VMT01 for MC1R-positive melanoma, remain in early-stage clinical development. The former Cesium-131 brachytherapy business is reported as discontinued operations under a pending divestiture.
Financial performance and position
Unless otherwise noted, amounts are in millions of dollars. Continuing operations had grant revenue but no reported product sales; therefore, product margins are not applicable.

The 2023 discontinued operation generated $6.936 million of sales and $0.463 million of gross profit before operating costs and a $4.170 million loss on classification as held for sale. A $2.651 million deferred tax benefit reduced the reported total net loss. The year-over-year comparison is not directly comparable: Viewpoint was acquired in February 2023, and 2023 included substantially higher development spending and merger-related costs.
At December 31, 2023, cash and cash equivalents were $9.238 million; restricted cash was $0.182 million. Working capital was $1.173 million and the current ratio was 1.07, versus $50.097 million and 17.07, respectively, at December 31, 2022. Total liabilities were $22.712 million, including $1.725 million of notes payable and lease liabilities. Net cash used in operations was partly offset by $24.373 million of investing cash inflows, primarily from maturity of short-term investments and cash acquired with Viewpoint. Cash, cash equivalents and restricted cash decreased by $11.755 million during the year.
Material changes, outlook and risks
- Portfolio and operations: The February 2023 Viewpoint merger shifted the company toward radiopharmaceutical development. The acquisition included $50.0 million of in-process research and development and $24.1 million of goodwill. The company agreed to sell substantially all of its brachytherapy business to GT Medical; closing was expected in the first half of 2024. The transaction terms include a 0.5% interest in GT Medical and potential royalties, subject to sales and other conditions.
- Clinical and operating outlook: Management expects substantially higher 2024 expenses, particularly R&D, as it advances VMT-α-NET and VMT01 and progresses preclinical programs. Both lead therapeutic programs had completed their first dose cohorts and were enrolling second cohorts as of the filing’s reported updates. VMT-α-NET has FDA Fast Track designation. In March 2024, the company announced a collaboration with Bristol Myers Squibb to study VMT01 with nivolumab. None of the company’s current candidates is approved for commercialization.
- Liquidity and financing: Management believes year-end cash together with subsequent financing and transaction proceeds will fund operations and capital investments into 2026. After year-end, the company raised approximately $69 million gross in a January 2024 public offering, received approximately $20.8 million from Lantheus for shares, and raised approximately $87.4 million gross in a March 2024 private placement; Lantheus’s $57.4 million participation was included in that March total, not additional to it. Lantheus also agreed to a $28 million one-time payment for option and negotiation rights. These equity raises materially increased shares outstanding and diluted existing holders. The company also paid $8 million for a New Jersey manufacturing facility in March 2024. These are subsequent events, not year-end cash balances.
- Key risks and contingencies: The company has a history of losses, no approved radiopharmaceutical products, and expects continued losses and potential need for further capital. Clinical outcomes, regulatory approval, patient enrollment, manufacturing scale-up, reimbursement and competition remain uncertain. Thorium-228 is sourced from a single Department of Energy supplier. Other risks include dependence on third parties, intellectual-property and licensing risks, radioactive-material handling and environmental obligations, and potential future equity dilution.
- Legal and reporting matters: A qui tam complaint concerning payments to a former medical director was dismissed without prejudice in January 2024; the U.S. and California authorities had declined to intervene or pursue action on those allegations. The company reported a $0.2 million estimated liability for separate stockholder litigation settlement negotiations. Management reported effective disclosure controls and internal control over financial reporting; the auditor issued an unqualified opinion on the financial statements but was not engaged to audit internal controls.
Important facts for investors to verify
- Confirm cash runway assumptions against actual spending, the timing and net proceeds of post-year-end financings, the $28 million Lantheus payment, and facility investment.
- Track share count, pre-funded warrants, options, ATM capacity and Lantheus participation rights to assess dilution and potential share supply.
- Review clinical-trial enrollment, safety, dose escalation and efficacy data for VMT-α-NET and VMT01; early or investigator-initiated findings are not proof of future benefit.
- Verify status and final economics of the GT Medical divestiture, including retained liabilities, contingent royalties and remaining brachytherapy-related obligations.
- Assess whether manufacturing capacity and Thorium-228 supply can support planned trials, and monitor licensing terms, regulatory developments and legal contingencies.
