Can AI Keep ISRG Ahead of Rising Robotic Surgery Competition?

Intuitive Surgical’s ISRG technology strategy is becoming increasingly important as competition in robotic surgery intensifies. The company is using artificial intelligence, software and data-driven capabilities to make its da Vinci ecosystem more useful beyond the core robotic platform.

In the second quarter, Intuitive Surgical highlighted the increasing adoption of My Intuitive Surgical+, an integrated da Vinci 5 offering that combines telepresence, simulation and AI-driven case insights. Although the initial renewal cohort was small, no customers opted out, suggesting early acceptance of the platform’s broader digital capabilities.

The company is also leveraging da Vinci 5’s computing capabilities to create a continuing stream of software enhancements. Intuitive Surgical has planned more than 100 updates, with the first phase focused on telepresence, simulation-based training and care-team workflow.

Several updates are designed to improve usability and efficiency, including tools that reduce communication between surgeons and care teams, enable multiple robotic arms to be adjusted before or during procedures, and provide a digital ruler for intraoperative measurements. Management believes these recurring upgrades can progressively increase the attractiveness of da Vinci 5 and support a multi-year upgrade cycle.

Force Feedback provides another layer of differentiation. Intuitive Surgical reported that a prospective study associated its use with faster return of bowel function following partial and radical nephrectomy. The study data showed that 63% of patients recovered within one day versus 28% without Force Feedback. Force Feedback instruments are excluded from the company’s extended-use program, underscoring their role as an innovation-driven component of the ecosystem.

AI-driven insights, continuous software development, simulation, workflow optimization and advanced instrumentation strengthen Intuitive Surgical’s competitive moat. Its nearly 13,000-system installed base and broad Gen 4 ecosystem provide a platform on which these innovations can compound, potentially keeping the company ahead of insurgent companies focused solely on robotic hardware.

Globus Medical GMED is moving beyond robotic navigation toward a broader data, analytics and AI-driven surgical intelligence ecosystem. The company is using data from patient treatment to learn and improve patient selection and surgical execution, with the goal of assisting surgeons rather than replacing clinical judgment. This strategy is being integrated with the Excelsius platform, where digital preoperative planning can be uploaded to ExcelsiusGPS and ExcelsiusHub for robotically navigated screw placement.

With more than 60 product-development projects underway, Globus Medical views AI as a long-term differentiator capable of connecting intelligent patient selection, proceduralization and complementary implants to improve surgical outcomes.

Stereotaxis STXS is taking a more direct approach to embedding AI into its robotic ecosystem through Synchrony and SynX, its digital surgery platforms. Management describes these systems as providing enhanced workflow, remote connectivity and smart AI capabilities, with Synchrony serving as a cockpit for robotic procedures as well as an independent opportunity in non-robotic operating rooms.

STXS plans to incorporate intelligent decision-support features into Synchrony and add automation to its robotic platforms. The company sees AI as part of a much longer digital-surgery roadmap, alongside telerobotics and broader automation. Stereotaxis believes these capabilities can ultimately expand the clinical and commercial value of its robotic platform beyond today’s applications.

ISRG’s Price Performance, Valuation and Estimates

Shares of ISRG have lost 33.6% so far this year compared with a 10.8% decline of the industry.

From a valuation standpoint, Intuitive Surgical trades at a forward price-to-earnings ratio of 31.8X, above the industry average. But, it is significantly lower than its five-year median of 67.91X. ISRG carries a Value Score of D.
