Can Spyre's SPY772 HS Study Broaden Its Growth Story Beyond Core IBD?

Spyre Therapeutics SYRE expanded its inflammatory-disease strategy in August with SPY772 and the phase II SKYLIGHT study in hidradenitis suppurativa, or HS. The move takes its combination approach into a new disease area beyond inflammatory bowel disease and rheumatology.

The opportunity is broader pipeline optionality, but the clinical bar is clear. SKYLIGHT must show that adding TL1A inhibition to an IL-17A/F backbone can improve outcomes enough to justify further development.

Spyre Brings Dual-Pathway Inhibition to HS

SPY772 combines SPY072, a long-acting anti-TL1A antibody, with SPY007, a long-acting anti-IL-17A/F antibody. The program extends Spyre's strategy of attacking more than one inflammatory pathway at the same time.

That approach is already central to its IBD pipeline, where pairwise combinations target alpha 4 beta 7, TL1A and IL-23. Moving the concept into HS gives Spyre a chance to test whether its combination thesis can travel beyond gastrointestinal disease.

SYRE's SKYLIGHT Trial Has a Clear 16-Week Test

SKYLIGHT is evaluating SPY072 plus bimekizumab against bimekizumab alone in approximately 150 adults with moderate-to-severe HS. The primary endpoint is the HiSCR75 response rate at week 16, with topline data expected in late 2027 or early 2028.

The design tests whether adding SPY072 to bimekizumab can improve the week-16 response rate relative to bimekizumab alone. Positive results could support further SPY772 development in HS and other indications.

Spyre Extends Its Platform Beyond IBD and Rheumatology

HS adds another disease area to a pipeline that already spans ulcerative colitis, rheumatoid arthritis, psoriatic arthritis and axial spondyloarthritis. Spyre is also advancing multiple IBD monotherapies and dual-antibody combinations, giving the company several mechanisms and readouts to evaluate.

Competition in HS is active. AbbVie ABBV has an established presence through Humira and is developing ABBV-1451, an IL-1 alpha/beta antibody for HS. Incyte INCY has an FDA-accepted New Drug Application for povorcitinib in moderate-to-severe HS, with a U.S. launch targeted for the first quarter of 2027 if approved.

SYRE Adds Optionality but Also Execution Demands

A broader pipeline reduces dependence on any one study, but it also raises the amount of clinical and manufacturing work Spyre must fund. Research and development expenses rose 63.2% year over year to $65.5 million in the second quarter of 2026 as manufacturing, clinical study and headcount costs increased.

Spyre ended the quarter with $1.15 billion in cash, cash equivalents and marketable securities. Management expects that position to fund operations into the second half of 2029, although the company is still expected to need significant additional funding as development advances.

SYRE's Scores Favor Watching the New Catalyst

SKYLIGHT gives Spyre another important proof point for whether its multi-pathway strategy can support growth beyond core IBD. The study is still early, and the larger investment case remains exposed to clinical development risk, rising cash burn and intense competition.
